We’re thrilled that you have chosen Dr. Sonia Rebeles, and becoming a patient with us is as easy as contacting our office! Our staff is trained to handle one-on-one initial consults to learn more about you and what type of care you may need. They will let you know if you’re contacting the right physician for your care, and if so, help you set an appointment with you at a time convenient for both you and Dr. Rebeles. The staff is not trained to give out or recommend medical advice, so be sure to understand that when you call, and allow them to help you set your appointment, and save your specific diagnosis and treatment questions for your appointment.

Appointment Preparations
We look forward to welcoming you to our office. Please arrive 15 minutes early to fill out paperwork. Once you arrive, let the receptionist know if you have any special needs, or need to visit the restroom. It is a good idea to write down your questions ahead of your appointment, and refer to your notes to make sure you don’t forget anything. Initial appointments may take more time, so plan plenty of time, and try not to rush your appointment.

Getting to know you and your medical history is of the utmost importance. For your greater convenience while visiting our office, please download, print and complete this two-page Patient History Form and bring it with you to your first appointment.

Transferring and Accessing Your Medical Records
Transferring your medical records to Dr. Rebeles should be handled prior to your initial appointment if possible. We suggest that you contact your primary care office, or previous GYN office, and ask them about their procedure in transferring records. Let our office know, upon making your initial appointment with Dr. Rebeles, whether you have already arranged transfer of your medical records from your previous physician’s office.
If you have not arranged transfer of your medical records and wish to do so, we can arrange a medical records request that is initiated from Dr. Rebeles’ office as well. Accessing Your Medical Records Online via the
Online Patient Portal
Having instant access* to your current medical records lets you access your medical information online, anytime, at no extra charge. You can see your own health care information and communicate electronically with your physician.
